If you’re looking to make a real difference in enabling someone’s independence and active lifestyle? We’d love for you to apply!
A former firefighter and dog lover is looking for your help to provide care. Taking care of/spending time with his dog, participating with the US archery team, and working a new home keep this member very busy. Sometimes trips with the archery team happen, where staff will come and help as well. This member uses a wheelchair, one-person pivot transfers, and external catheter throughout the day. If you enjoy easygoing conversation, a pet friendly home, and providing a hand to live an active and engaging life, this could be a wonderful fit for you!
Make a Meaningful Impact with Community Horizons
Support individuals. Empower independence. Build a flexible career you’ll love.
Community Horizons proudly supports individuals across Wisconsin who are enrolled in self-directed programs through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the CLTS (Children’s Long-Term Support) Program.
Self-direction allows participants—and families—to have control over who provides their care and how services are delivered, creating a truly personalized experience. While Community Horizons is not the employer of record, we assist by connecting caregivers with participants and guiding them through the hiring process. All hiring decisions are made by the member or their guardian
