Going Out: Taking her on errands or outings using the provided vehicle.
Why You're Great For This Role (Requirements):

You are a deeply reliable and consistent person - when you show up, it directly impacts her independence.

-
You are 18 or older.
-
You have a valid driver's license and a reliable vehicle.
-
You can pass a Wisconsin background check

-
You are comfortable in a pet friendly home (dogs/cats)
You'll be working one-on-one with a young Triplegic woman in her De Pere home who is looking for a reliable, long-term addition to her support team. Bonus points if you're a fellow night owl! She loves staying up late for movie marathons, which means her Sunday morning routine starts a little later in the day (10AM start time!).
This is a perfect, steady position for a student, someone looking to supplement their income, or a seasoned caregiver who wants a low-stress, highly rewarding 12-hour weekend shift. You answer directly to the client, not a corporate agency. This is a unique, direct-hire opportunity through a secure Medicaid funded self-direction program.

Make a Meaningful Impact with Community Horizons
Support individuals. Empower independence. Build a flexible career you’ll love.
Community Horizons proudly supports individuals across Wisconsin who are enrolled in self-directed programs through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the CLTS (Children’s Long-Term Support) Program.
Self-direction allows participants—and families—to have control over who provides their care and how services are delivered, creating a truly personalized experience. While Community Horizons is not the employer of record, we assist by connecting caregivers with participants and guiding them through the hiring process. All hiring decisions are made by the member or their guardian
