Duties

• Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and cath care
• Assist with meal preparation
• Maintain a clean and safe environment by performing light cleaning duties

Who is Community Horizons?

Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.

Self-direction allows participants to make their own choices, selecting who provides their care and how those services are delivered.

While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.
