At Community Horizons, we are always striving to connect care workers with their neighbors who need a little extra help. The individuals we work with take part in Self Directed Supports (SDS); a Medicaid funded program that offers them the possibility to employ care workers of their own choosing. You read that right; employ. An SDS care worker is employed by the member they support. Members get to choose who is coming to provide the supports that they require to comfortably and safely stay in their homes.

Community Horizons proudly supports individuals across Wisconsin who are enrolled in self-directed programs through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the CLTS (Children’s Long-Term Support) Program.
Self-direction allows participants—and families—to have control over who provides their care and how services are delivered, creating a truly personalized experience. While Community Horizons is not the employer of record, we assist by connecting caregivers with participants and guiding them through the hiring process. All hiring decisions are made by the member or their guardian
