At Community Horizons, we are always striving to connect care workers with their neighbors who need a little extra help. The individuals we work with take part in Self Directed Supports (SDS); a Medicaid funded program that offers them the possibility to employ care workers of their own choosing. You read that right; employ. An SDS care worker is employed by the member they support. Members get to choose who is coming to provide the supports that they require to comfortably and safely stay in their homes.

Are you a helper at heart? Do you have a few hours each week to help a neighbor in their home? Community Horizons is hoping to connect you with a Shawano area woman who is seeking some supportive home care assistance
Tasks:
- Sweeping and mopping floors
- Dusting
- Washing bedding at the laundromat
- Assistance with errands
- Light snow removal
Qualities:
- Motivated

- Work well independently
- Reliable

- Physically capable of performing cleaning tasks
- Valid driver's license and vehicle
- Comfortable in a pet friendly home - two small dogs
