Home Support Caregiver - Oshkosh

Hours: 16 per week
Pay: $16 per hour + mileage reimbursement

Community Horizons is hiring a reliable, supportive home care worker to assist a disabled man in his home. He has a great sense of humor and enjoys fishing, and he requires consistent, dependable help to maintain his daily routine and independence.

Responsibilities

• General cleaning
• Taking out trash
• Vacuuming and dishes
• Driving him to appointments
• Assisting with grocery shopping and errands

• Able to provide steady, reliable support

Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.

Self-direction allows participants to make their own choices-selecting who provides their care and how those services are delivered.

While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.
