About the Role

This position provides one‑on‑one support in a calm, respectful home environment. The ideal caregiver is compassionate, reliable, and comfortable supporting someone who values independence, routine, and clear communication.

Responsibilities May Include

• Assistance with morning routine
• Light housekeeping or meal prep
• Companionship and conversation
• Support with mobility or transfers (ceiling Lift)
• Medication
• Ensuring safety and comfort throughout the shift
• Straight Cath ( Will Train)

Requirements

• Valid driver’s license (if transportation is part of the role)
• Ability to pass a background check
• Reliable transportation

Who is Community Horizons?
Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.

Self-direction allows participants to make their own choices—selecting who provides their care and how those services are delivered.

While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.
