About Community Horizons LLC

Make a Meaningful Impact with Community Horizons
Support individuals. Empower independence. Build a flexible career you’ll love.

Community Horizons proudly supports individuals across Wisconsin who are enrolled in self-directed programs through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the CLTS (Children’s Long-Term Support) Program.

Self-direction allows participants—and families—to have control over who provides their care and how services are delivered, creating a truly personalized experience. While Community Horizons is not the employer of record, we assist by connecting caregivers with participants and guiding them through the hiring process. All hiring decisions are made by the member or their guardian

Ready to Make a Difference?

If you’re passionate about helping people live as independently as possible in their own homes, we’d love to talk with you. Community Horizons offers the support you need to succeed in your self-directed role.
