Community Horizons is assisting several local households in Wisconsin Rapids and Stevens Point area in finding compassionate, dependable individuals to join their Home Care Team. This role is perfect for someone who enjoys helping others live safely, comfortably, and independently in their own homes.

You’ll support individuals with a variety of needs—including seniors, adults with disabilities. Your presence helps clients feel valued, supported, and empowered.

What You’ll Do

• Assist with Activities of Daily Living (ADLs): bathing, dressing, grooming, mobility
• Monitor general well‑being and observe changes in condition
• Prepare simple, nutritious meals and assist with meal planning
• Support safe transfers using Hoyer lifts or other assistive devices
• Offer companionship, conversation, and emotional support
• Perform light housekeeping: cleaning, laundry, tidying
• Follow safety protocols and respond appropriately in emergencies

What Makes You a Great Fit

• Experience in home care, assisted living, group homes, or long‑term care
• Comfort using mobility equipment such as Hoyer lifts
• Ability to lift, support, and assist with transfers
• Strong communication skills and a calm, compassionate presence
• Experience supporting individuals with developmental disabilities or autism is a plus
• Reliable, respectful, and committed to high‑quality care
