What We’re Looking For:
· 3-5 years of relevant experience
· Capability to pass a caregiver background check
· Valid driver’s license, auto insurance, and a clean driving record are required.
Hours: $20/hour
Work Schedule:
M-T-W-Th-F 4pm-10pm (e/o Thursday) 8am-4pm; Opportunity to pick-up on the weekends to meet 40hrs per week.

What You’ll Do:
· Assist with daily living activities, including personal hygiene and independent life skills.
· Prepare delicious meals that cater to her dietary requirements.
· Act as her coach and cheerleader, helping her set and achieve goals while promoting independence.
· Engage in enjoyable recreational activities and outings to keep her connected and active.
· Maintain a cozy and clean home environment through light housekeeping duties.
· Ensure safety and well-being by adhering to policies and documenting her care.

Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.
