Why This Job Matters
Your presence gives his family the respite they need — and gives him a caregiver who sees him, supports him, and helps him grow. If you love adorable kids, meaningful work, and a role where every shift truly counts, this is the job for you.
If you’d like to bring your skills, compassion, and energy to this wonderful family, we’d love to hear from you.
Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.
Self-direction allows participants to make their own choices-selecting who provides their care and how those services are delivered.
While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.
Our mission is to help individuals of all ages navigate self-directed care with connection, coordination, and compassion.
