Community Horizons – In-Person Position

A Christian family partnered with Community Horizons is seeking a compassionate respite/personal support provider who shares their values. You will support four children—two boys (age 10) and two girls (ages 8 and 10)—in a home where kindness, respect, and healthy boundaries are emphasized. This is a pet‑friendly household.
Weekdays after school
Occasional weekends
3:30 PM – 7:30 PM, three days per week
Approximately 4 hours per shift
Potential for additional hours
Communicate with parents about plans and activities for each shift
Provide homework assistance
Support and mentor children through evening and bedtime routines
Engage in age‑appropriate activities:
With the boys: games, Legos, cars, creative play
With the girls: conversation, homework help, games, and bedtime routines
Ability to pass a background check
Reliable transportation
Kind, friendly, patient, and aligned with Christian values
Experience supporting children or adults with behavioral challenges
Experience working with children with autism
Experience supporting children who have been in out‑of‑home placements or adopted

Community Horizons offers support for individuals enrolled in self-directed programs through managed care organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record; members make all hiring decisions with the involvement of third-party fiscal agents responsible for payroll.
