Community Horizons – In-Person Position

A Christian family partnered with Community Horizons is seeking a compassionate respite/personal support provider who shares their values. You will support four children—two boys (age 10) and two girls (ages 8 and 10)—in a home where kindness, respect, and healthy boundaries are emphasized. This is a pet‑friendly household.

🕒 Schedule

• Weekdays after school

• Occasional weekends

• 3:30 PM – 7:30 PM, three days per week

• Approximately 4 hours per shift

• Potential for additional hours

🧸 Responsibilities

• Communicate with parents about plans and activities for each shift

• Provide homework assistance

• Support and mentor children through evening and bedtime routines

• Engage in age‑appropriate activities:

  • With the boys: games, Legos, cars, creative play

  • With the girls: conversation, homework help, games, and bedtime routines

• Ability to pass a background check

• Reliable transportation

• Kind, friendly, patient, and aligned with Christian values

• Experience supporting children or adults with behavioral challenges

• Experience working with children with autism

• Experience supporting children who have been in out‑of‑home placements or adopted

Community Horizons offers support for individuals enrolled in self-directed programs through managed care organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record; members make all hiring decisions with the involvement of third-party fiscal agents responsible for payroll.
