Wage: $14.38-$20.00/hour
Schedule: When needed – Evenings and weekends

Community Horizons is seeking an individual to provide dependable respite care for their family .An 8-year-old adolescent female on the autism spectrum. She prefers to keep to herself but enjoys engaging in games on her phone such as Minecraft. 2 additional children, ages 2 (in diapers) and 6, may also require care at times.

Responsibilities include:

·       One-on-one supervision and engagement with adolescent daughter

·       Caring for multiple children when needed

·       Light meal preparation for the children

·       Diaper changes and basic care for toddler

·       Providing a safe, calm, and supportive environment

Requirements include:

·       Reliable and dependable caregiver

·       Evening and weekend availability

·       Reliable transportation

·       Experience caring for children, especially those with special needs

·       Comfortable around pets

Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.

Self-direction allows participants to make their own choices—selecting who provides their care and how those services are delivered.

While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.
