Job description:
Community Horizons is partnering with a dedicated family in the Appleton area to find a community support provider for their four children (ages 6–11), all of whom are on the autism spectrum and have unique strengths and support needs. We are looking for 2 staff members, so the ratio will be 2:1.*
This is an active, hands-on role that involves taking one child at a time into the community for fun, structured outings. You'll be responsible for planning activities, providing transportation, and supporting the child during each outing.

Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.
Self-direction allows participants to make their own choices—selecting who provides their care and how those services are delivered.
While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.
