A family in Neenah is seeking dedicated Nurses to join a warm, supportive care team in Neenah, WI. If you’re a skilled nursing professional who leads with compassion, reliability, and heart, this family would love to meet you.
Why This Role Stands Out
You’ll be providing 1:1 overnight care for a gentle, medically fragile 2‑year‑old. Nights are typically calm and quiet—he sleeps soundly—but your presence ensures safety, comfort, and peace of mind for the family.
This is meaningful work with a steady rhythm, ideal for nurses who appreciate focused, one-on-one care.
Schedule
Full-Time:
Sunday–Wednesday
7:00 PM–7:00 AM or 8:00 PM–8:00 AM
(Times may shift slightly based on family needs.)
The family understands that great nurses have busy lives, so they’re open to discussing availability. If you’re interested, they’re willing to talk through options.
What You’ll Do
Mom describes this as a “breezy, low-stress shift”—as long as you’re awake, alert, and ready to act when needed.
Compensation (Medicaid-funded; wage set by program guidelines)
What You Bring

Support individuals. Empower independence. Build a flexible career you’ll love.
Community Horizons proudly supports individuals across Wisconsin who are enrolled in self-directed programs through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the CLTS (Children’s Long-Term Support) Program.
Self-direction allows participants—and families—to have control over who provides their care and how services are delivered, creating a truly personalized experience. While Community Horizons is not the employer of record, we assist by connecting caregivers with participants and guiding them through the hiring process. All hiring decisions are made by the member or their guardian
If you’re passionate about helping people live as independently as possible in their own homes, we’d love to talk with you. Community Horizons offers the support you need to succeed in your self-directed role.
