Community Horizons proudly supports individuals across Wisconsin who are enrolled in self-directed programs through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the CLTS (Children’s Long-Term Support) Program.
Self-direction allows participants—and families—to have control over who provides their care and how services are delivered, creating a truly personalized experience.
