A private employer in the Plover community is looking to add a caregiver to his care team! This is not your typical caregiving role — it’s perfect for someone who enjoys staying busy and having fun. The adult individual you’ll support loves being on the go and participating in a wide range of activities, including watching NASCAR and Monster Jam, fishing, bowling, walks in local parks, board games and mini-golf.
He’s extremely social, enjoys spending time with friends, and participates in adaptive sports like bowling, track, and bocce ball throughout the year. Your role will be to support him in staying safe, comfortable, and fully engaged in the activities he loves — both at home and out in the community.
Schedule:

Support individuals. Empower independence. Build a flexible career you’ll love.
Community Horizons proudly supports individuals across Wisconsin who are enrolled in self-directed programs through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the CLTS (Children’s Long-Term Support) Program.
Self-direction allows participants—and families—to have control over who provides their care and how services are delivered, creating a truly personalized experience. While Community Horizons is not the employer of record, we assist by connecting caregivers with participants and guiding them through the hiring process. All hiring decisions are made by the member or their guardian
If you’re passionate about helping people live as independently as possible in their own homes, we’d love to talk with you. Community Horizons offers the support you need to succeed in your self-directed role.
