Community Horizons is seeking a dependable, patient, and energetic childcare provider to support a fun 15‑year‑old with special needs and a psychiatric disorder.

5 to 40 hours per week
After school: starting at 1:00 PM
Weekends: starting at 10:00 AM The family is flexible and open to discussing any availability you can offer.
$17.98 per hour
Plan and participate in community outings
Keep the child’s bedroom clean, organized, and maintained
Spot clean the bathroom (surface cleaning, trash, restocking)
Provide transportation to and from activities
Use de‑escalation strategies when needed
Offer 1:1 supervision
Create a safe, supportive environment for community exploration
Valid driver’s license
Reliable vehicle and willingness to use it for local travel
Experience supporting individuals with complex needs (mental health, developmental disabilities, autism, ADHD, ODD, etc.)
Willingness to learn, accept coaching, and grow
Ability to pass a Wisconsin Caregiving Background Check
Strong dependability and commitment to supporting individuals with special needs
Note: This is a cat‑friendly and dog‑friendly home, with additional small caged pets. No animal care is required, but important for those with allergies or fears.
| Community Horizons offers support broker services for individuals enrolled in self-directed programs within IRIS (I, Respect, I, Self-Direct) or Family Care throughout Wisconsin. As a Support Broker, we offer services to help plan and direct self-directed support. How can we help you? Community Horizons assists community members with the recruitment process of personal caregivers. Position range from a few hours per week to full-time positions and offer flexibility which often works great for college students. Go to school to learn and grow while fulfilling your passion by helping others. Self-Direction is a unique program that allows community members within the state program remain as independent as possible. With your help you can play a big part in keeping this possible. Self-Directed caregivers are hired directly by the members which often allows for a more flexible schedule and competitive pay.
