After‑School Caregiver – Appleton

Pay: $18.00–$18.48/hr
Schedule: Monday–Friday, 2:30 PM–7:30 PM

Community Horizons is hiring a reliable caregiver to support a 14‑year‑old boy in his home. The routine is steady, the expectations are clear, and you’ll be the person who helps his afternoons run smoothly. Helping to provide guidance with daily living skills.

Job Duties

• Meet him after school when he gets off the bus
• Make simple snacks/meals and cut food into bite‑sized pieces
• Support him with utensil use
• Assist with toileting, showering, and dressing
• Take short outdoor walks (fresh air encouraged)
• Guide him through his evening routine and bedtime prep

• Comfortable with hands‑on personal care
• Able to follow a consistent routine
• Someone who doesn’t mind a job where small wins matter

Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.

Self-direction allows participants to make their own choices—selecting who provides their care and how those services are delivered.

While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.
