Community Horizons is assisting a local community member with the recruitment process of a self-directed caregiver. We are looking for someone special to fill a very vital role in someone's life! Make a difference while helping someone in your community. So much more than a caregiving job, we are looking for someone who is an advocate at heart.
Wage: $14.25-$18.83
Schedule: Mondays, Wednesdays, and Fridays 10 AM-4 PM FLEXIBLE
Job Duties included but not limited to;
-General Housekeeping tasks
o Dishes
o Bathroom/Kitchen Tasks
o Take out garbage
o Sweeping/Mopping
o Organization
-Laundry and linen change
-Cooking and assistance with feeding depending on the day.
-Errands/grocery shopping/community activities
-Personal Care Assistance
□ Standby Transfers as needed
□ Bathing,
□ Toileting assistance
□ Feeding as needed
-Companionship- Help with making phone calls, and advocating members’ needs and wishes.
- Assistance within the community with doctor appointments and errands.

Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.
Self-direction allows participants to make their own choices—selecting who provides their care and how those services are delivered.
While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.
