A vibrant and spirited woman in Wisconsin Rapids needs your heartfelt assistance to continue living independently in her apartment. Are you driven by passion, dedicated to making a difference, and thrive on building a genuine connection with someone on a personal level? Do you have a flexible schedule to fill in as needed? Current Need
- Flexible caregiver ready to step in when needed for any of the regular in-home shifts
Daily In-Home Schedule (for reference only)
- 8:30 am – 10:30 am
- 5:30 pm - 7:00 pm,
- 10:30 pm – 11:45 pm.
Wage $16.32 / Hour
Inspiring Tasks
- Gently assist her in rising each morning to embrace the day ahead.
- Help her unwind and settle into bed for a restful night
- Provide compassionate personal care, including transfers and toileting
- Prepare light meals and maintain a clean, comforting environment.
Ideal Candidate
- Experienced and compassionate with personal care and transfers
- Familiar with the unique needs of individuals with cerebral palsy.
- A lover of pets—both dogs and cats—who can share the joy of life with furry companions.
- Must have a reliable vehicle for transportation.
- Favorable Caregiver Background Check
Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organization or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.

Self-direction offers you the work/life balance everyone is yearning to achieve. In a self-direction role, you will be hired directly by the individual you are caring for making it a true private-in-home position. This means working directly 1:1 in a private home setting, with no forced shifts and a schedule agreed upon by you and your member.
