Job Title - Mentor – Make a Difference (and Have Fun Doing It!)
Location: Appleton, WI
Pay: $14.39/hour
Schedule: One weeknight (5–7 PM) + 2 hours on Saturdays between 9 AM–3 PM

We’re looking for a mentor for an energetic 13-year-old with autism who loves people and always wants to be on the move. This is a chance to make a real impact while having fun outside and building a meaningful connection.
Requirements:
· Must be at least 17 years old
· Have a valid driver’s license and reliable transportation
· Enjoy being active and engaging with others

Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.
Self-direction allows participants to make their own choices-selecting who provides their care and how those services are delivered.
While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.
