PT Housekeeping & Support
Are you someone who combines empathy with strength and clear communication? Do you know how to support others with kindness—while maintaining your own boundaries and professionalism? If you’re dependable, grounded, and confident in handling challenging dynamics, we’d love to connect with you.
We’re looking for a steady, reliable individual to provide light housekeeping and in-home support for a person living independently through the self-direction program. This is a part time non-agency position, meaning you’ll work directly for the individual you’re supporting. You’ll have the opportunity to build a one-on-one relationship where your consistency, honesty, and emotional maturity will truly make a difference.
This position involves supporting someone who has high expectations and who benefits from structure, respect, and clarity in communication. The right person will bring both compassion and confidence—someone who can stay calm under pressure, hold boundaries kindly but firmly, and take pride in creating a supportive, well-organized environment.
This role is ideal for someone who:
· Communicates clearly and maintains strong personal boundaries
· Handles challenging situations with patience and professionalism
· Enjoys meaningful, flexible work with purpose
· Is reliable, grounded, and self-directed
If you bring both heart and backbone to your work—and know how to balance care with confidence—this could be an excellent fit.
Pay: $16.32 per hour + $0.35/mile reimbursement for errands
Schedule: Mon, Wed and Fri afternoons, 9 hours per week
Additional hours may be available temporarily

Community Horizons proudly supports individuals across Wisconsin who are enrolled in self-directed programs through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the CLTS (Children’s Long-Term Support) Program.
Self-direction allows participants—and families—to have control over who provides their care and how services are delivered, creating a truly personalized experience. While Community Horizons is not the employer of record, we assist by connecting caregivers with participants and guiding them through the hiring process. All hiring decisions are made by the member or their guardian
