Schedule: M, Tue, Thurs, Fri  (10a-2p)

Plus! One Saturday a month for fun activities!

Wage: $16.32 per hour, plus mileage

Tasks:

·         General Housekeeping

·         Meal Prep/Bulk Cooking

·         Laundry

·         Grocery Shopping/Errands

·         Companionship

·         Fun Activities

Ideal Candidate:

·         Caregiving Experience (1 year preferred)

·         Calm, Caring and Patient

·         Experience working with individuals with Autism (1 year preferred)

·         Must be able to pass background check.

·         Valid driver's license and reliable vehicle

Who is Community Horizons?
Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program. Self-direction allows participants to make their own choices, selecting who provides their care and how those services are delivered.

While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.
