Are you compassionate and reliable? One of our wonderful members is seeking a part-time weekend employee to provide care in her Pulaski area home.
Short Shifts – Friday through Monday – Late morning to early afternoons
Duties:
· Assist with bathing
· Hoyer transfers
· Repositioning
· Catheter care
· Meal preparation
· Housekeeping
· Companionship
· Assist with errands
Pay Rate: $17.00/hour
This is direct in-home care—no agencies, no facilities – the person you support is your employer.
Who is Community Horizons
Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.
Self-direction allows participants to make their own choices—selecting who provides their care and how those services are delivered.
While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.
