Community Horizons has the honor of helping a family find a reliable caregiver for part time hours in Kimberly! Don’t worry if you don’t have extensive healthcare training, on-the-job training is provided for all tasks. This is the perfect part-time position, Great hours, Great pay, Great family to work for!
Schedule: Saturday/Sunday 8:30AM-11:30AM (Up to 1:30PM)
PRN: 3 Weekdays 2:30PM-5:30PM
Pay Rate:$20/hour
Care Needs:
- Range of motion stretches
- PT assistance- The Caregiver will track BP.
- Meal Prep.
- Bathing assistance
- Dressing/undressing assistance.
- Straight Cath. 1-2 times per shift (Training provided).
- Bowel Program (Training provided).
- Use of a Hoyer lift/sit-to-stand lift during each shift.
- General housekeeping.
- Medication assistance.
Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.
Self-direction allows participants to make their own choices—selecting who provides their care and how those services are delivered.
While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.
