Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record but instead, we are connecting applicants to members and assisting with navigating the hiring process. Members make all hiring decisions. 

Self-direction offers you the work/life balance everyone is yearning to achieve. In a self-direction role, you will be hired directly by the individual you are caring for making it a true private-in-home position. This means working directly 1:1 in a private home setting, with no forced shifts and a schedule agreed upon by you and your member.
